Choosing the best breast implant size is the most difficult decision that our patients face when having breast augmentation surgery. At Austin-Weston, the plastic surgeons do not choose the breast implant for you. What they do is listen to you, educate you on the pros and cons of the choices of the different breast implants, and allow you to choose what you feel will give you the best shape and size for what you want to accomplish. The goal is to make certain that you get a beautiful breast, not just a larger breast.
There are many issues to consider with breast augmentation. Some women want to be as large as they can be, and some women want a smaller breast implant mostly for shape – not size. Sometimes it comes down to a competition between shape and size. That is, at some point a larger breast implant is less shapely and less attractive – and is more likely to have complications than a smaller breast implant. As an example, a large breast implant that gives a full cleavage and upper breast fullness can shift the nipple position down and make it look unnatural. This might look good in a low-cut dress, but not look good out of clothing. Our plastic surgeons will look at your anatomy and listen to what you want, and tell you what is surgically possible. It is not always straight forward because there are many choices, and each one has a particular consequence.
The two ways patients are ‘sized’ for breast implants is by using sizers and by using 3-D imaging. Based on your goals and anatomy, you and your doctor will come up with some sizes that might look good on your frame. You will then get to try on sizer implants under your clothing to see what that size of implant will look like in clothing. Often this is the best way to get a true feel for what an implant will look like in your body.
We also use VECTRA 3-D imaging to help you choose the right size breast implant at your complimentary breast augmentation consultation. This special camera creates a virtual 3-D image of your body, and different sizes of breast implants can be placed virtually to see how the results can look in real life. Bringing in before-and-after photographs of other women can be helpful to illustrate what you like and don’t like. These photos should be nude, and show consistency with regard to looking at the camera and arm placement. Photographs of women who you like their look, but are in a bikini, are not illustrative because a bikini can conceal imperfections, or add support to what could be a very droopy breast outside the bikini.
The definition of what looks ‘real’ and what looks ‘fake’ comes down to what naturally exists in the world. If there are women walking around in the world with natural breasts that are the size and shape that you like, then you are likely looking for a natural and ‘real’ look. If you like a very round, high-cleavage or ‘push-up bra’ look even without a bra, then you probably like a ‘fake’ look. Both are possible with the right implant, so being clear with your plastic surgeon regarding what you like and what you don’t will make sure you are happy with your result.
Currently, the FDA has approved both silicone gel and saline breast implants as safe and effective options for breast enhancement. Breast implants have been on the market for decades and we have thnds of satisfied patients.
All breast implants are composed of a durable silicone shell, and then either filled with saline or silicone. Both saline and silicone breast implants have specific advantages and depending on your goals, your doctor may recommend one over the other. In general, saline implant feels slightly harder and is more prone to rippling, but the appearance is very round, and saline implants cost less. Saline implants can also be placed through a smaller incision because the implants are placed inside the body when deflated, and are then inflated with saline once the implants are in the right spot. This also allows the surgeon to choose exactly how much volume goes inside the implant.
Silicone implants are softer and offer a more natural feel, though they cost slightly more. In general, there is less rippling with a silicone implant. This can be relevant if you are very then and do not have much breast tissue to camouflage any rippling.
Both saline and silicone implants can be placed via all types of incision and can be placed either above or below the muscle. 80% of women who come to Austin-Weston choose a silicone breast implant over saline.
During your consultation, one of our board-certified cosmetic plastic surgeons will go over the most common incision types for breast implant placement. The patient wishes, scar and healing capability, implant size and associated complications will all factor into your final decision. The most common incision type is the IMF (infra-mammary fold) incision, which lies in the natural fold beneath the breast. Other options include the areola incision which is placed at the junction of pigmented areola skin to less-pigmented breast skin. The least popular option is the armpit incision where the breast implants are placed though a remote incision in the armpit. We have lots of before-and-after photographs to illustrate the different scar options and lots of staff members who have had the procedure done and are happy to show you how well-hidden a good breast augmentation scar can be.
The human breast consists of skin, fat and glandular tissue, all over the top of the pectoralis muscle, which rests on the ribs. The breast implant is either placed above or below the pectoralis muscle, depending on your wishes and goals for your final look. The most common placement is under the muscle, as the muscle creates a gentle slope to the top of the implant, and a less ‘done’ or ‘fake’ result. The muscle essentially camouflages the top of the implant, making for a natural and soft breast. For patients wanting a more ‘fake’ and augmented look, a saline breast implant placed above the muscle gives an extremely round and projected appearance. For certain patients who are very physically fit, it sometimes makes sense to place the implant above the muscle as well. This can be done with a smaller silicone implant for a natural look, or with a larger saline implant for a ‘fake’ look. The decision is ultimately up to you and your surgeon, but rest assured the experienced plastic surgeons at Austin-Weston routintely perform hundreds of breast augmentations per year, using varying combinations of implants, incision types and muscle placements and will help you choose the right procedure.
